You have entrusted us with your visual health. Our doctors and staff take the responsibility very seriously. As you know, our practice utilizes the latest in technological innovation to stay on the forefront of eyecare. A large amount of our annual budget goes to the two most important aspects of our company: our people and technology.
On January 22nd 2017 VisionQuest Eyecare, discovered that a cyberattack occurred in our network. You are receiving this letter because your name is in our database. It is possible that your private information including name, address, phone number, date of birth, social security number, health or vision insurance information, medical claims data and clinical information (Private Health Information) may have been compromised as a result of this cyberattack on our network. At this point there is no evidence that any of the above information was in fact compromised. We are notifying you, however, out of an abundance of caution.
Since this discovery, it has been our highest priority to further secure our network and data. We have invested in multiple technology solutions in order to mitigate further risk of a data breach.
We apologize for the inconvenience this may cause you. We urge you to monitor your credit history carefully over the course of the next few months. Freezing your credit with the three major US credit agencies is a good way to prevent potential fraudulent activity. There is no cost to freeze your credit.
